      I was searching for some information and came across your website referring to ‘dialysis facilities’. I was very much impressed with the information you provide and would like you to be aware that I post, to my website http://www.qualitysafepatientcare.com the dialysis facility survey (inspection) reports that are conducted by the California Department of Public Health on behalf of Centers for Medicare and Medicaid Services (CMS) which is the overisght agency for these facilities. These surveys clearly show the real care that is delivered in facilities, along with other data that is required of facilities to report

      Dear Sir/Madam,

      I made the report about Polymyositis and the adverse effect of TD Adsorbed vaccine.
      You have quoted me exactly. However, at the end, you state that I was hosptalized (true), and ‘patient recovered’, (completely untrue!). Please remove that phrase. I am still struggling with numerous problems (myalgia, myositis, related auto-immune problems. I am mostly bedridden, in a wheelchair, and have suffered tremendous amount of atrophy in my proximal core muscles, including the hips, lower back, thighs, upper back and shoulders.

      I am glad you are providing this information. It would be great if you remove the phrase (patient recovered) (what a joke), and add the information that I have given above.
