It's well known to most, and should be to any professional, that drugs can poop out after awhile. He could increase dose to see if his tolerance has increased, or he can just try a new drug. If he has problems with Aggression, he may do better with a mood stabilizer than an antidepressant, or by adding a mood stabilizer to his Lexapro.
