After drug was administered, patient experienced the following problems/side effects: abnormal dreams, amnesia, anxiety, completed suicide, confusional state, depression, gun shot wound, hallucination, mood altered. LIDODERM dosage: unknown. During the same period patient was treated with TALWIN NX, AMBIEN, LORCET, LORTAB, NORCO, VICODIN, TEMAZEPAM, DALMANE. Patient died on 09/21/2004.
