After drug was administered, patient experienced the following problems/side effects: completed suicide, incorrect dose administered. LIDODERM dosage: unknown. During the same period patient was treated with HYDROCODONE BITARTRATE, CHANTIX, KLONOPIN, PROTONIX, ASCORBIC ACID, VITAMIN D12, PAXIL. Patient died on 09/25/2007.
