Darvon Removed From the Market

In my March 2006 editorial (), I reported the headline in USA Today that the public advocacy group Public Citizen petitioned the Food and Drug Administration (FDA) to remove propoxyphene (Darvon) from the market. In the March 2010 editorial (), I reported that the FDA planned to further evaluate the safety of propoxyphene with the Centers for Medicare and Medicaid Services and the Veterans Health Administration. Looking more closely at the history of this drug, it is quite surprising that it took 53 years to remove it from the market despite a number of reports, case studies, and research reporting its dangers. Darvon Is Still in the Public Eye

In June 2006, I wrote an editorial about a consumer group urging the Food and Drug Administration (FDA) to ban the drug Darvon (). On July 7, 2009, the FDA released information about several actions to reduce the risk of overdose in patients using Darvon or Darvocet (). These actions were taken because of data linking propoxyphene and fatal overdoses. To further evaluate the safety of propoxyphene, the FDA plans to work with the Centers for Medicare and Medicaid Services and the Veterans Health Administration to study how often the elderly are prescribed propoxyphene over other analgesics. Since 1957, propoxyphene (Darvon) and its products have been available for use as a supplementary analgesic for mild to moderate pain. I have prescribed Darvocet-N 100 and other Darvon compounds for moderate pain resulting from rheumatoid arthritis, osteoarthritis, and other painful musculoskeletal and rheumatic disorders for many years with considerable success. Undesirable and adverse side effects are usually minimal in both incidence and severity. Abstract: We used a large medical insurance claims database to identify three groups: chronic opioid use (>180 therapeutic days, N=3726); acute opioid use (63%) in the chronic pain group and used a much greater share of all medical services than males, especially as they grew older. Refractory Restless Legs Syndrome Likely Caused by Olanzapine

We report a case of severe restless legs syndrome (RLS) that occurred as a side effect of olanzapine therapy. It was refractory to treatment with 2 mg of clonazepam and 3 mg ropinirole. There was partial relief with propoxyphene, however, it was stopped because of side effects. The symptoms disappeared once olanzapine was switched to another antipsychotic medication. Only two prior published reports associate olanzapine usage with development of RLS. In one report, low-dose benzodiazepines and ropinirole were associated with resolution of RLS symptoms stating dopamine depletion as the likely etiology. Conclusions: Always inappropriate and rarely appropriate medication use decreased over time in this practice-based research network study. Gabapentin as a Potential Option for Treatment of Sciatica

We describe two patients with sciatica who were successfully treated with gabapentin. The first was a 32-year-old man with severe shooting pain in his left leg that was later diagnosed as sciatica secondary to a fifth lumbar-first sacral intervertebral disk herniation. The patient was treated with acetaminophen, nonsteroidal antiinflammatory drugs (NSAIDs), narcotics, and muscle relaxants; he reported only limited pain relief with any of these agents or combination of agents. He was then prescribed gabapentin 300 mg once/day; his pain substantially improved, even after the first dose. The drug was titrated gradually up to 900 mg 3 times/day with good results. Cognitive effects of opioids

This article summarizes the biomedical evidence addressing cognitive effects of the opioid analgesics. Current evidence indicates that cognitive function can be influenced by use of opioid analgesics, although the effects vary between drugs, are thought to be most significant with mixed-activity drugs, codeine, propoxyphene, and meperidine, and are generally most concerning during the first few days after starting opioid therapy, before tolerance develops. Blanket policies regarding the activities of driving and working are inappropriate; this issue is best addressed on a patient-specific basis. Analysis of pain management drugs, specifically fentanyl, in hair: Application to forensic specimens.

This article discusses the immunoassay screening of pain management drugs, and the mass spectrometric confirmation of fentanyl in human hair. Hair specimens were screened for fentanyl, opiates (including oxycodone), tramadol, propoxyphene, carisoprodol, methadone, and benzodiazepines and any positive results were confirmed using gas chromatography or liquid chromatography with mass spectral detection. The specific focus of the work was the determination of fentanyl in hair, since autopsy specimens were also available for comparison with hair concentrations. Using two-dimensional gas chromatography with electron impact mass spectrometric detection, fentanyl was confirmed in four of nine hair specimens collected at autopsy. Inappropriate prescribing in an older ED population.

The objective of this study was to determine the prevalence of potentially inappropriate medication (PIMs) use in older emergency department (ED) patients based on the updated 2002 Beers criteria. This was a retrospective analysis of 352 consecutive ED visits by patients aged 65 years and older. The mean number of medications taken was 8.4 per patient. In the study population, 111 (32%; 95% confidence interval [CI], 27-36) of 352 patients were taking at least 1 PIM at ED presentation. Propoxyphene use and risk for hip fractures in older adults.

CONCLUSIONS: The results of this cohort database study suggest that propoxyphene use among adults aged &gt; or =65 years is associated with increased risk for hip fracture and suggest a need for interventions to reduce propoxyphene use in older adults. Clinicians should be aware of the risk for hip fracture with other opioids as well and weigh them against potential benefits when prescribing for older adults.
