"Patient reported eruption of one hive like lesion (wheel) morning after vaccination, followed by other ""pimple like"" area over her body. No puritis except for the first lesion. INFLUENZA (FLUZONE) problem was reported from VA on Jan 22, 2007. Male patient , child 3 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 15, 2007. "Patient was asleep by 7:30pm on 1/10/07. Around 8:15 he started crying and screaming in his bed and was restless. Mom went in to get him and picked him up. His body would tense up and go stiff and flat as a board for about 5-10 seconds and then release. About 30 seconds to a minute later he would repeat this stiffening again. During the first episode this lasted for about 15 minutes. The whole time he was going through this mom was holding him in her arms while they say on the couch. He would repeatedly call out "" I want mommy"" as if he didn't even recognize that mom was right there and he was in moms arms. He was not cognizant of what was going on around him at all. As soon as it was over he went right to sleep as if nothing happened. This same thing went on at 9:30 and 11pm. It may have happened a fourth time, mom can't remember but the last time was at 11:15pm. Each time that it went on the reaction time was shorter. By the final time it lasted only about 3-5minutes. Every time he would wake up from his sleep crying and thrashing around very disoriented. He continued to go through the contraction and stiffening episodes and calling out I want mommy yet not recognizing that he was in mom's arms. He would not respond to anything that you said to him or asked him. If I asked him where he hurt or to point to where he hurt, he did not respond at all.". . NONE. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from NC on Jan 22, 2007. Female patient , child 0.9 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 12, 2007. "Rash on legs, fever, twitching to left. No fever with tick spells. Jerks 3-4 x hour happened with focusing on something. Referred to neurologist. Observation was only treatment done. Patient is better with ticks present. 1/26/07 Received neuro consult report which reveals patient seen on 12/7 for EEG & 12/8 for complete neuro eval. Neuro exam was WNL. A video tape of the patients jerking movements was reviewed. She rapidly twisted her head to the left & then returned back to neutral position. No LOC, no facial expression change or any other associated movements. No loss of eye contatct or focus & is developmentally on target. FINAL DX: Motor tic disorder; noted to be highly unusual for child of this age & probably better described as movement disorder.". NONE. Allergic to formula Family & birth HX: negative for tics. Positive for mental retardation in paternal second cousin who was preemie & also blind & deaf. Patient was 7 lb 4 oz 38week delivery w/o complication. Breast fed & allergic to form. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from NC on Jan 24, 2007. Male patient , child 1.3 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 10, 2007. Flu vaccine given 12/27/06 at 1:30. Mother stated felt warm and fussy that day at 5:00. On 12/28 at 2am had fever and gave Tylenol and seemed ok when woke up that am was running and fell on milk bottle in his mouth. Hit nose. Immediate started seizure and carried to ER by rescue squad. ER had difficulty controlling seizure. Child was airlifted to medical center. Talked with mother on 1/10/07 states child was ok now. Not on any medications. No more seizures. No problem with any prior immunization. Child did have seizure once before at 11 months of age. It was very brief and did not require MD visit. Mother informed MD of this. Brother has febrile seizure.. NONE. NONE. Patient was hospitalized Patient recovered.

"Patient stated about 3 hours after receiving flu vaccine developed shortness of breath, wheezing and a rash on his abdomen and chest. The above is a physician and he self treated with a nembulizer treatment which improved respiratory symptoms. The rash persisted for 1-2 weeks.". NONE. Allergic to Zithromax and Clindamycin. "MGM reports noticing blister with small white (left arm-back) head on 01/14/07. On 01/16/07 the blister opened and ""bled a little"". Area is red, warm to touch, firm knot. "Patient came in to clinical 1/11/07 for a flu shot and a pneumonia vaccine. Pneumonia vax given in left deltoid. 4-5 hours after vax given, left arm began to swell and turn red. Arm painful from site to elbow. Symptoms lasted 4-5 days.". . Diabetes. Patient recovered.

"After flu shot was received patient had a sore arm, soreness from fingers to shoulders. Ibuprofen has relieved pain for short periods of time, pain has persisted to present day.". . INFLUENZA (FLUZONE) problem was reported from MT on Jan 24, 2007. Female patient , child 2.0 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 24, 2007. "Received influenza immunizations on 11/10/06 (Lot #U2255 AA given by LPN) and 12/21/06 (administered by RN). She had very mild cold symptoms (slight rhinorrhea) early in January 2007. On 1/13 she developed ataxia, fatigue and leg pain. She had then progressive weakness and pain (legs/arms/back) and then frank refusal to bear weight. She was hospitalized on 1/16/07. She had an LP and nerve conduction velocities done on 1/17. LP with elevated protein and no pleocytosis and EMG revealed prologed nerve conduction velocities. She was diagnosed with Guillain-Barre Syndrome and received 5 days of IVIG. She was discharged on 1/24/07 still with weakness and refusal to bear weight, but her truncal ataxia was improved as was her pain. She was discharged home on Neurontin and with physical therapy.". None. None. Patient was hospitalized Patient recovered.

INFLUENZA (FLUZONE) problem was reported from IL on Jan 24, 2007. Male patient , 22 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 24, 2007. The subject is a nursing student who received the flu shot during a simulated disaster drill type process. He received the flu shot on 10/26/2006 at 1215. At 1230 he returned to the clinic area stating that he was lightheaded and nauseated. He also stated that his righ arm had a tight feeling and was numb and tingling. At 1240 his face became very flushed and hot to the touch. He did not have and respiratory distress. The occupational health physician said to send him to the ED or give him an epi-pen. He refused the ED so an epi-pen was provided and administered at 1300. After about 15 to 20 min his face became less red and hot to the touch. His lightheadedness and dizziness got better as well as the arm numbness and tingling. His father picked him up and took hime to his regular doctor for follow-up since he had refused going to the ED. He returned to school on 10/30/2006 with a full doctor's release with no restrictions.. None. None. Patient recovered.

Sites free from any s/s of infection or irritation/ VSS no temp. Patien informed by PA-C to increase fluids, take tylenol/motrin for the bodyaches, RTC if sx increase or any other complication arise.". none. . Patient recovered.

Client sick to stomach within 30 minutes after receiving the vaccine. Later developed red blotches on face and hands and generalized itching. Given Benadryl 50 mg orally with diminishing of symptoms. Vomited small amount of pinkish saliva/mucous about 5 minutes after the Benadryl was given. Advised seek medical attention if symptoms recur and continue Benadryl for 24 hours.. None. None. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from FL on Jan 29, 2007. Male patient , 42 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 22, 2007. "Initial information received from a health care professional on 19 January 2007. A 42-year-old male patient a first dose of Fluzone SV 2006-2007, lot number U2253AA, on 05 December 2006, the patient developed numbness and tingling in his feet. By 17 December 2006, his back hurt and he was fatigued. He was seen in an emergency room the following day and was admitted to the hospital because he could not walk. On 20 December 2006, the patient was transferred to the intensive care unit (ICU) and was treated with immunoglobulins and plasmapheresis. His right side was paralyzed, and he was in severe pain. He was treated with Percocet. He was diagnosed with Guillain-Barre Syndrome. The patient has a past medical history of poorly controlled diabetes. On 03 January 2007, the patient was transferred to a rehabilitation facility after having been hospitalized for 16 days. His recovery status is unknown.". Unspecified Cholesterol and Diabetes meds. "The patient has a history of diabetes. According to the reporting physician, the patient is ""not in good control of diabetes"". Patient was given steroid, antibiotic and Benadryl in ER. On 1-8-07 patient reported back to ER for complaining of tingling sensation from elbow to chest. Patient also states they're scared to sleep. "This is a 4-year-old white female who presents to the emergency department after having a flu shot and was started on Premarin cream today for labial effusion. Second dose was at 10:30 p.m. About 45 minutes later the patient complained of being cold with her legs hurting, throat hurting, having headache and chills according to the mom. She had no fevers. She has had no rash or wheezing. No hives, and now all has completely returned to normal with no complaints at all.". . "Keflex, Sulfa". Patient recovered.

"Patient called at 1430 stating development of approx 20 hives on face, arms, chest, and belly within one hour of receiving flu shot. Took Allergan and Ventolin Inhaler at 1030 and inhaler again at 1430. Chest feels tight (like asthma), denies numbness or tingling in lips or tongue. Encouraged Benadryl (if able to take it) and if sx not resolved by hs encouraged ER/Ambulance for difficulty breathing and/or numbness or tingling of lips, tongue, etc.". Allegra prn/Ventolin inhaler prn. environmental allergies (seasonal) and asthma. Patient recovered.

No mass, no redness or warm to area.". None reported. "NKDA, None reported, States he is very healthy. Previous prostate problems.". Patient recovered.

"Flu vaccine given 12-9-05, seen in ER 12-21-05 for seizures - starring and jerking which mom now realizes started shortly after flu vaccine given. Has been followed by Neuro. Test reveal no cause for delay and hypotonia.". None at that time. Head small for age-5th percentile. Patient was hospitalized Patient recovered.

"vaccine administered 1/23/07-no initial reaction. Today 1/25/07 RA red, swollen, warm to touch, itchy down to elbow.". NONE. NONE. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from CA on Jan 31, 2007. Male patient , child 2.4 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 30, 2007. "Hives. Zyrtec, Epi.". . . Patient recovered.

INFLUENZA (FLUZONE) problem was reported from PA on Jan 31, 2007. Female patient , 89 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 27, 2007. "The next morning fingers swelled and pain on arm of shot, went across chest into other arm. Starting to effect severe pain and swelling every muscle and joint in body. Unable to walk or pick up any object with arms. Low fever. 2/2/07 Received medical records from PCP which reveal patient seen 1/17/07 for diffuse muscle pain. Had been evaluated in early Nov for complaints of leg pain & dx then with arthritis of hips & lumbar spine. Started PT but got progressively worse with pain extending to arms with severe muscle pain & stiffness of neck, shoulders, arms & lower legs, esp worse in mornings. Patient had unexplained weight loss of about 5 #. Exam revealed diffuse muscle tenderness to palpation and unsteady gait requiring use of assistive device. Tx w/steroids & if no improvement was to be referred to rheumatology. Daughter related that patient received flu shot at pharmacy which reported to daughter they had 15-17 cases of serum sickness reaction from same lot. FINAL DX: possible serum sickness reaction; although polymyalgia rheumatica type syndrome was felt to be more likely.". . "NONE PMH: arthritis, unexplained weight loss". Patient recovered.
