INFLUENZA (FLUZONE) problem was reported from IL on Jan 10, 2007. Female patient , child 3 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 10, 2007. "Varicella vaccine received on 1/04/07 along with influenza vaccine. Mother reported seeing a couple spots on his chest, back, and diaper area on 1/06/07. Mother stated rash increased on 1/07/07, but it was totally covering his chest, back, and diaper area on 1/08/07 with itching. Child was taken to family MD on 1/08/07, and physician verified the rash as varicella. Treatment was for symptoms: Tylenol was ordered for temperature, and Benadryl liquid for itching.". None. None. Patient recovered.

AFTER RECIEVING THE FLU VACCINE THAT SHE HAD SWELLING OF THE LIPS WITH SOME ITCHING. PT. CAME FOR THE FLU VACCINE AT APPROXIMATELY 4:30 P.M. AND DURING DINNER TIME SWELLING WAS NOTICED. THE HELATH DEPT. WAS CLOSED BY THEN . PT. DID NOT GO TO THE ED BUT RETURNED TO THE HEALTH DEPT. ON 12/20/06 @ APPROXIMATELY 9:30. THE SWELLING WAS REPORTED AS DECREASED WHEN SHE ARRIVED- LOWER LIP NO LONGER SWOLLEN BUT UPPER LIP SLIGHTLY SWOLLEN. PT. REPORTED NO ALLERGIES TO EGGS OF MEDICINE ON THE REGISTRATION FORM AND AT THIS RETURN VISIT. BP 98/64 AND WT. 74 LBS. GIVEN BENADRYL 25 MG.. NONE. GESTATION AT BIRTH WAS 35 WEEKS. Patient recovered.

In am of 1/10/07 patient was noted to have a 4x4.5 cm swelling on her left arm at the site of injection. No redness, fever, or other symptoms were noted. She received a flu vaccine last year with no problems.". Dilaudid PCA Oxandrolone 2.5 mg orally daily Famotidine 20 mg orally twice daily Norvasc 5 mg orally daily Catapres TTS-1 patch weekly. SX lasted approx 3 days and resolved. 5 days ago 12 weeks after flu shot, had urticarial rash, red and raised that came and went away with Benadryl, but back with warm, back and since has had sporadic urticaria on back, chest, and arm, no resp SXS.". . . After receiving Pneumonia vaccine on 11/30/06 in left arm became very painful at injection site and has been painful since then. Has been taking Tylenol and it relieves for couple of hours. No redness or temp. Advised to take Benadryl and continue taking Tylenol. Cool cloths as necessary.. . . Patient recovered.

Gave Preservative-Free Fluzone to child younger than 6 months of age thinking the patient had to be 6 weeks to receive the flu vaccine. Observed child about 45 minutes following the shot and no allergice reaction had occurred. Called mom the following day (today) and mom has not noticed any abnormalities in her daughter such as difficulty breathing or redness/swelling at the injection site.. . . Patient recovered.

Reddness and swelling extended from left shoulder to elbow. Patient seen on 1-8-07 by Dr. Patient diagnosed with pneumovax reaction with secondary cellulitis. Keflex 500 tid given. Seen by Dr. on 1-12-07 - patient has decreased redness and cellulitis. Patient under care of Dr.. "Synthroid, Cardizem, Diovan, Nexium, Allegra, Nitroglycerin, Effexor, Ativan, Fibermax". "Hypertension, Irritable Bowel, Hyperthyroidism, Anxiety". Patient recovered.

She has always gotten her yearly flu shot with no complications, but felt tired and/or lethargic weeks after this years vaccination. Her daughter relayed all this info to me. 1/26/07 Received medical records from hospital which reveal patient experienced respiratory distress & was admitted 1/8-1/11/07. Treated w/antibiotics, bronchodilators & steroids. Improved significantly & was d/c on tapering prednisone & close f/u. FINAL DX: Multilobar pneumonia in a smoker; possible bronchogenic neoplasm which will be worked up as outpatient; hyponatremia; COPD; multifocal atrial tachycardia, short duration; tobacco abuse; & hyperlipidemia.". unknown. unknown COPD; continues abuse smoke; hyperlipidemia. Mild at first. It settled in the muscle on the underside of the arm. Became very painful. Especially 1st thing in am. Went to doctor 12-13-06. Prescribed Ibuprofen 800 mg, didn't help. Prescribed Sanindoc 200 mg, did not help December 22 December 28 Methocarban 500 mg no help and then Hydrocol Apap 5 500 mg. On 1/2/07 went to orthopedic physician. X-rays showed nothing. Gave me a cortisone shot. It worked and pain went away. Will see him again 1/30/07. He could not tell me if the flu shot was to blame. I called the above service and asked what was in my shot. Have not heard back still would like to know. Do I get another shot next fall?". . . Patient recovered.

"Child reportedly had low grade fevers day after vaccines and received ibuprofen. About 48 hours after vaccines fever spiked to 105 and child had seizure lasting about 2 minutes, post-ictal about 15 minutes. Transported by ambulance to emergency department. MR received from hospital visit of 01/07/2007 for c/o febrile seizure 2 days post vax.. HPI: Child began running fever day of vax. Parents using Motrin but underdosing for weight. Day of seizure child went limp and then began jerking and this lasted about a minute. In ER temp was 105.1F. Dropped to 102.5F with Tylenol and Motrin. She was D/Cd home from ER on Tylenol and Motrin for fever control. Discharge DX: Febrile Seizure.". NONE Meds: Motrin.. NONE PMH: none. Allergies: none.. Patient recovered.

"On 11/16/06, the patient was given a flu shot at a clinic. She pointed to an area of the lower deltoid where she said the shot was given. She had family visit her after Christmas and was sleeping on the sofa. She developed pain and redness of left shoulder and was seen by an orthopedic surgeon, who did a left shoulder X ray which showed tendonitis. Dr said it was due to her sleeping on a sofa.". "Pamelor, Zoloft, Cardizem.". "Penicillin, Hypertension, depression.". Patient recovered.

INFLUENZA (FLUZONE) problem was reported from OH on Jan 17, 2007. Female patient , 49 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 17, 2007. "Pt received inactivated Influenza vaccine, 0.5ml IM in the left deltoid at 1410 on 16 Jan 2007. At approximately 1508, the pt came to the front clinic desk c/o wheezing/coughing. I quickly notified Dr who came out and brought her to the back treatment room. He listened to her lungs, hten requested us to perform a PFT on the patient. After the PFT's were accomplished, she was administered a nebulizer treatment of Albuterol 5mg and Atrovent 0.05mg. Following completion of the neb tx, Dr listened to her lungs again, and requested us to repeat the PFT. After those results came up,the patient was still complaining of cough. Dr asked her several times if she had any other symptoms other than a cough, andthis time she said she was having some chest tightness. Dr ordered me to administer 0.3mg of Epinephrine IM. I administered the Epi at 1542. Following the dose of epi we obtained vital signs. After the second set of vital signs, the patient was given 40mg of Prednisone to decrease the chances of a latent phase reoccurance of the reaction. We observed the patient for an hour, she continued to improve, then when Dr felt confident enough about her condition, she was released to her husband who drove her home.". . "Allergic to Amoxicillin, sulfa drugs and tetracycline. Significant chronic hx of asthma, ARD, SAR.". Patient recovered.

Had been well for week prior to vax & no sick contacts. Temp in ER was 102.3. Tx w/antipyretcis in ER. D/C to home w/o further seizure activity. FINAL DX: Viral syndrome & possible febrile seizure.. NONE. NONE. Patient recovered.

"Febrile Seizure approximately 10.5 hours after vaccination. 1/22/07 Received medical records from reporter which reveal patient seen 1/18 for flu shot & was well at that time. Seen again on 1/19 s/p ER visit for febrile seizure. Exam at that time found bulging red bilateral TMs. Treated w/antibiotics. 1/26/07 Received medical records from hospital which reveal patient experienced an apneic spell with cyanosis, congestion & seizure activity at home. On exam, right ear TM reddened. Given 1 dose of antibiotic. No further seizure activity in ER & was d/c to f/u w/PCP next day. FINAL DX: Febrile seizure.". Robitussin CF. Cleft lip s/p repair.. "After patient completed clinical exam at approximately 3:50pm. Patient given flu vaccine. After taking a few steps down hallway to leave at approximately 4pm patient complained of burning, itching and feeling broken out on face. Patient noted to have red, flushed cheeks. Right cheek appeared to have a circular area about dime size with red outer area and white inner area, over Right outer eyebrow noted to be red with a few small bumps. Doctor saw patient and advised to give patient Benadryl. Approximately 4:03pm patient given Benadryl elixir 50 mg (4tsp). 4:25pm cheeks appear more normal with just a few pinkened areas, circular area on Right cheek barely visible. Area over Right eyebrow normal in appearance with only a small bump noted. Patient reported face didn't feel like it was burning or itching anymore. Patient left with mom.". Depo-provera 150 mg IM Left deltoid at approx 3:52pm. No known allergies and no known medical conditions.. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from TX on Jan 22, 2007. Female patient , 29 years of age, was vaccinated with INFLUENZA (FLUZONE) on Dec 20, 2006. "Redness, induration of injection site 45 mm-tightness of throat, fever 101F, chills, localized pain, nausea, vomiting, diarrhea.". NONE. UNK. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from VA on Jan 22, 2007. Male patient , child 12 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 11, 2007. Received present complaint from Emergency Room Doctor requesting name of vaccines child received 2 hours in ER. Seizure this AM in school for 2 minutes.. NONE. NONE noted on screening questionnaire. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from NC on Jan 22, 2007. Female patient , child 1.3 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 12, 2007. "Approximately 9 hours after receiving MMR (fist) and influenza vaccines patient experienced a generalized tonic seizure lasting less than 2 minutes. No treatment was necessary. No past medical or family history of seizures noted. 1/23/07 T/C to reporter to request missing info. Parents called EMS for patient & called PCP but no treatement done & did not transport to ER. Seen by PCP following day. 1/25/07 Received medical records from PCP which reveal patient had decreased growth velocity in all parameters, slightly worse in weight w/o evidence of chronic disease. Patient eats well but is fussy & has had several URIs & otitis media infections. On day of initial flu vax 11/9/06, patient noted to have bilat OM & was treated w/antibiotics & referred to ENT. Seen again on 12/19 with thrush from antibiotics & mild cough. Next visit was on 1/11 & received flu vax & MMR after well check up. Seen s/p seizure on 1/12 & found to be well with no further seizure activity. No family hx of seizures. No treatment done & no further w/u recommended unless seizure recurs.". . NONE. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from GA on Jan 22, 2007. Female patient , child 12 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 18, 2007. Approximately 10 minutes after administering the Fluzone vaccine child began complaining that her arms and back was itching. I noticed red whelps on her back and chest. Physician was notified immediately and the patient was given Allervert (antihistamine) for allergic reaction. Fluzone was received 30 minutes after other vaccines.. "Albuterol, Metered dose inhaler". Penicillin allergy. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from AZ on Jan 22, 2007. Female patient , child 7 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 08, 2007. Localized swelling next day. Induration 45x25mm. 90x50 erythema. Follow up redness only.. "Benadryl over the counter, Dimetapp for cold". . Patient recovered.
