INFLUENZA (FLUZONE) problem was reported from AR on Feb 15, 2007. Female patient , child 1.1 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 04, 2007. "Parents report temp ""110 degrees"" had generalized ""shaking"" was given one shot, Tylenol, and prescription for 1 dose qd for 5 days. Seen at facility.". . NONE. Patient recovered.

INFLUENZA (FLUZONE) problem was reported from NC on Feb 15, 2007. Female patient , child 01 years of age, was vaccinated with INFLUENZA (FLUZONE) on Feb 14, 2007. "Initial and follow up information has been received from a physician and an office manager concerning a 12 month old white female patient with no past medical history who on 15-DEC-2006 was vaccinated with a dose of HPV rL1 6 11 16 18 VLP vaccine (yeast). Concomitant vaccinations on the same day included a first dose of measles-mumps-rubella vaccine (MSD) SC into the left leg, Hib conj vaccine (OMPC) (manufacturer unknown) and a first dose of varicella virus vaccine live (MSD) SC into the left leg. Other concomitant vaccinations included a first dose of influenza virus split virion 3v vaccine inactivated (FLUZONE) (lot# 427U7AA) IM into the right leg. It was reported that the patient received HPV rL1 6 11 16 18 VLP vaccine (yeast) by accident. She was supposed to receive a dose of pneumococcal 4 6B 9V 14 18C 23F conj vaccine (CRM197) (PREVNAR). The reporter stated that the products were clearly labeled; there was not a confusion of products. It was ""lack of education"" of the medical assistant working on the office. The parents reported the patient had slight fever on 16-DEC-2006 (also reported as the 17-DEC-2006). Subsequently, the patient was seen in the office on 18-DEC-2006 and had recovered from slight fever. There were no there reported problems or ""any other adverse events."" Patient reported adverse reaction to health dept on 1/16/07 c/o trouble breathing-use of own inhaler more often, hives/rash ""all over body""-use of Benadryl cream. Severe itching, on 1/19/07 seen by PCP-Dr, NP. Rash better, breathing better, and no other treatments by NP, patient recovering.". "Albuterol neb inhaler, Norvasc, Nexium". INFLUENZA (FLUZONE) problem was reported from UT on Feb 20, 2007. Female patient , 42 years of age, was vaccinated with INFLUENZA (FLUZONE) on Jan 30, 2007. "Our client received vaccines January 16, 2007. On January 22 she noticed a rash on her stomach, which moved on her back and up into her armpits. It was still moving when she called on January 25th. She had no hives, no fever, no chills. I spoke with the PA-C, who said it sounded like a histamine reaction and we advised her to try an antihistamine or see a doctor or go to the emergency room, if it concerned her. Also, she and another person, who received vaccines at our clinic the same day, both had a tooth that had become sensitive. We advised her to see a dentist.". Cymbalta. "This case was reported by a physician and described the occurrence of increased liver enzymes in a 4-year-old male subject who was vaccinated with Havrix for prophylaxis. Concurrent vaccination included influenza virus vaccine given on 27 January 2007. Concurrent medications included Amoxicillin trihydrate and Dimetapp-DM. On 27 January 2007 the subject received 1st dose of Havrix at 720 elisa units. At an unspecified time after vaccination with Havrix, the subject experienced increased liver enzymes and lethargy. This case was assessed as medically serious. At the time of reporting the outcome of the events were was unspecified. The physician considered the events were possibility related to vaccination with Havrix.". INFLUENZA (FLUZONE) problem was reported from CT on Feb 21, 2007. Female patient , 78 years of age, was vaccinated with INFLUENZA (FLUZONE) on Feb 21, 2007. "Stated that injection ""felt different"". No immediate pain, burning, numbness, tingling, tenderness or other symptoms upon or immediately after injection. No symptoms experienced until at least ? 2 days later. The arm bothers her in the left deltoid area (very localized)upon raising. No ""pain"" upon rest or with some types of movement - only with certain types of movement. Had ""rotator cuff"" problem in right arm and it may feel something like that but no awareness of injury to left arm. Is unable to characterize the ""pain"". Did not report or seek treatment from her private physician because it ""costs too much money"". She stated that she did not try to self-treat with heat, cold or medication. She states it has gradually improved but was concerned because of the length of time since receiving. She may mention to her PMD during her routine checkup in May 2007.". "Developed hives on left lower arm (same arm as shot given in), developed a cherry red mark on left and right jaw line and red dots on upper cheek bones-both left cheek and right cheek-mom gave Benadryl for treatment.". She states Pablo received his second flu shot on 12/12/06 and was admitted the next day to the hospital with a very high fever. He has had no previous reactions with any of his other immunizations, including the 1st flu shot on 11/14/06.". . none. Patient recovered.

"Initial report received on 09 February 2007 from a consumer, who is the patient. A 55 year old male received on 25 September 2004 Fluzone (lot number not reported) (side, site and side of administration were not reported) and within 15 minutes developed nausea and weakness that progressed to the right side of the face, right side of the neck and right upper extremity with numbness tingling and pain, which continue to present. Patient was able to retain mobility but was loosing strength. It was unknown if the patient was hospitalized. The patient was diagnosed with MS (multiple sclerosis) by a neurologist and did not recover. The patient was allergic to penicillin. CT scans and Spinal Tap were done. Results were not provided. The patient refused to provided all requested information.". . Allergic to penicillin. Patient recovered.

"Patient had received a 025 ml dose of Fluzone IM on 2/09/07. Should have received 05 ml. Phone cell to dept of health advised repeat dose of .5 ml which was given on 2/20/07. Mother called 2/21/07 to report a lacey like rash developed about 7pm on arms, legs, buttock and noticed it on abdomen and back am 2/21/07. Child has a low grade fever, rash not raised, no other SX recommended Benadryl and Tylenol for age.". . NKA. Patient recovered.

At 1650 pt c/o extreme itching and SOB. AT 1705 pt reports increase in itching, and swelling around eyes. Eyes red and puffy. Eppy 1:100 0.5ml SQ. Albuterol sun, Benadryl 50 mg IV, Solumedrol 125 mg x 4 doses given.". Meds: Cyclobenzaprine and Lortab.. "12/1/06 C/O stiffness and tenderness in her ""muscle"" at base of neck, posteriorly and between shoulders. 12/2/06 Pain became severe and worse yet on 12/3/06. 12/4 and 12/5: still ""very sore but a little better."" Tylenol, Motrin and heating pad didn't help. States injection site not sore, no redness, did not develop a fever. No problem with previous flu shots. Instructed to try applying ice to area, but to call private provider is discomfort is severe. Recovered approx. December 27th, 2006.". unknown. Patient complain of pain in her arm at injection site. She had quarter size redness and swelling which resolved in 2 or 3 days after the injection but she still has pain. "Whole body, bright red, warm, itching-breaking out of hives, legs starting to swell-given Benadryl 25 mg po qd sent to hospital ER admit time 4:00-called office with complaints 5:45 (approx. vac time 4:30-4:45)". . INFLUENZA (FLUZONE) problem was reported from TN on Mar 02, 2007. Female patient , 29 years of age, was vaccinated with INFLUENZA (FLUZONE) on Feb 20, 2007. "Pt received the flu shot on 9 Dec, 2006 and on 9 Jan 2007 RT side of her jaw had a constant pain. On Jan 17 she developed slurred speech. Went to ER on Jan 18 and discharged on the 20th. Next she had L foot numbness that spread into her chest. Could not walk without assistance. Was transferred to medical center for more tests. Diagnosis was Sub acute encephalomyelitis due to the influenza vaccine. 03/05/07-records received from facility for DOS 01/27-02/03/07-DC DX: subacute encephalomyelitis due to influenza vaccine. HX:1/9/07 right jaw ache, 1/17/07 acutely developed slurred speech, seen in ER 1/18/07, admitted. Brain scan and EEG at that time normal. 1/20/07 developed left foot numbness spread to right side of belly and chest, right hip tender to touch, bowel urgency and 'panic attacks' three times over 5 days. Difficulty walking and changes in her hand writing. PE: speech slurred, cranial nerve VII nasolabial flattening noted on smile, no facial asymmetry on eyebrown rise or eyelid closure. Cranial nerve XII tongue deviates to right on protrusion, decrease strength on right lateral deviation of tongue. Motor:no atrophy/drift. normal tone, 4/5 strength on ankle dorsiflexion on right side otherwise 5/5 throughout. Sensation:pinprick was decrease 40% on left side up to mid chest and upper back. Inconsistent response on proprioception testing on distal right lower extremity. Vibration intact. Reflexes:DTR 2 plus and symetric. Plantar responses flexor. Gait:native was unstable and with short steps, unable to walk on toes, heel or heel to toe.". . records received 7/13/07-Family HX father had MI at age of 19.. Patient was hospitalized Patient recovered.

Started with a rash that resembled eczema and spreaded all over his body within a week after a flu vaccination containing thimerosal. Behavioral he became hyper and listless.. . "His previous eczema was eradicated using alternative medicine; however, it came back after the vaccine. Previously he was tested for food allergies and came back negative, including eggs.". "Mother reports rash on patient's face all over with redness, swelling, itching starting 2-4 weeks after immunizations. Condition is persistent and mother reports worsening over time. "Patient was asleep and awoke approximately 3 hours after vaccination with tremors. Over then next 2-3 wks, developed Left sides weakness, numbness, tingling. Developed Left leg limp. Approximately 3-4 wks after vaccine, developed Right upper arm weakness along with worsening Left sided symptoms. Had migraine headaches, developed stuttering, numbness and tingling in face. Complain of feeling diaphramatic symptoms stuttering worsened with increased fatigue. Weight loss of 25 lbs. Treatment - under care of primary physician, doctor name reported. Plan has been: bedrest, nutritional and herbal supplements from a nutritionist, chiropractic care, specialist and hospital. IgG taken orally, ""Super ESS"" for re-myelination of nerves. Complain of her mother had a ""bad reaction"" many years ago to a shot. Mother required hospitalization.". NONE. "NONE, c/o ""sensitivity"" to medications". Patient recovered.

"Onset 1/17/07 of weakness in legs (""rubbery feeling""), numbness in hands progressing to biceps - T-102 - neuro consult with diagnosis of Sensory Guillain Barre - in ICU on ventilator and receiving IVIG. In rehab 2-5 ---> 2-28-07: discharged with diplopia; right 7th cranial nerve palsy; dysarthria; dysphagia and decreased mobility. 03/19/07-records received from facility for DOS-01/18/07-02/01/07 and acute rehabilitation from 02/05-02/28/07 DC DX: Guillain Barre Syndrome DC DX: Neurological disorder with acute inflammatory demyelinating polyneuropathy ""Guillain-Barre Syndrome"". Right VII cranial nerve palsy. Diplopia. Dysphagia. Dysarthria. Steroid versus ICU psychosis, resolved. Depression/Anxiety. Pulmonary issues, status post pneumonia, History of COPD and pulmonary involvment of AIDP. Decreased mobility. Decreased ADL. Admission to acute inpatient rehabilitation. Presented with C/O numbness in hands and weakness in legs. Noted a slight cough running nose and watery eyes recently. Temp of 102. Admitted. Continued to have muscular weakness, ataxia and difficulty with ambulation. Intubated and extubated.". UNK. NONE records received 3/19/07-PMH:Hypothyroidism. Telangiectasia. Hyperlipidemia. Impression:febrile convulsion or adverse reaction to immunization of flu vaccine. Bronchitis.". . . Patient was hospitalized Patient recovered.

Email from patient (coworker)to county nurse on 2/15/07 stating, ""Right after I got the tetanus shot, the arm I got it in got really sore which I expected. Unfortunately, the pain hasn't gone away and I'm having problems straightening my arm out. I know I didn't injure the elbow or strain it but I just can't get rid of the pain in the elbow and now the wrist is starting to bug me."" We referred the individual to her primary care physician. pc to patient on 3/5/07 found that patient did not seek medical attention. She states now she occasionally needs to pick her left arm up with her right one to move it.". Zocor and Avalide. "Hay Fever, hypertension, hyperlipidemia". Patient recovered.

INFLUENZA (FLUZONE) problem was reported from NY on Mar 12, 2007. Male patient , 16 years of age, was vaccinated with INFLUENZA (FLUZONE) on Mar 07, 2007. "Encephalitis, Seizures hospitalized at medical center, including ICU stay. 3/23/07 Received medical records from hospital which reveal patient experienced vomiting, fatigue, weakness, facial & chest petechial rash. Patient lived in dormitory at school, home on weekends. Seen by PCP who sent to outlying ER for w/u. While in ER patient had seizure & was admitted to ICU. Had another seizure in ICU & was transferred to higher level of care on 12/7/06-1/5/07. On admit exam showed not responsive to commands but opening eyes to voice & pain. DTRs absent. +Babinski on R & - on L. Consults by neuro, ID & neurosurg. Tx w/antibiotics, antivirals, multi antiseizure meds & IVIG. Experienced rash & all meds d/c. Then seizures recurred & antiseizure med restarted x 1. Developed mild bone marrow suppression thought to be due to antiviral med. Patient began to respond on 12/20 & gradually improved to almose full function by d/c on 1/5 to outpatient rehab. FINAL DX: Encephalitis.". . . Patient was hospitalized Patient recovered.

INFLUENZA (FLUZONE) problem was reported from OH on Mar 13, 2007. Female patient , child 1.0 years of age, was vaccinated with INFLUENZA (FLUZONE) on Mar 12, 2007. "Prolonged febrile seizure > 30 minutes lifesquad to ER, seizures meds, infant transferred to Children's ER, admitted to ICU. 03/26/2007 MR received for a 2 day hospital stay following transfer from local facility to pediatric hospital. Infant had a 1 day hx. of tactile fever, mild congestion, and decreased activity. Pt. noted by mom to have whole body shaking of generalized tonic/clonic type. Seizure activity >30 minutes. Initially treated at primary facility with Diastat and Ativan with subsequent apnea resulting in intubation . PE: ETT in place. Skin slightly mottled. Cool extremities. Intermittant 4 extremity twitches and shivers. Babinski decreased. Otherwise WNL. Txd with antibiotics, NS fluid bolus for decreased perfusion with good results, Phosphenytoin for seizure control. No further seizure activity observed. Extubated and weaned to RA without difficulty. Initial Assessment: Febrile Seizure likely 2' to MMR vaccine. Final DX: New Onset Seizure.". Meds: None.. NONE. PMH: None. No previous seizure hx. Allergies: None.. Patient was hospitalized Patient recovered.

INFLUENZA (FLUZONE) problem was reported from OH on Mar 13, 2007. Male patient , child 1.2 years of age, was vaccinated with INFLUENZA (FLUZONE) on Mar 13, 2007. "Client in for well child visit and did have 2nd flu vaccine (first administered on 1/9/07 by geneva dillon RN using lot #U2175HA). The lot number for the 2nd flu shot was UT2176BA. The 2nd flu shot was administered in the am before 12pm. Accordind to Mother, the child awoke from sleep at about 10:30pm with altered LOC. The child was also starting to seize with jerking movements. At the time the child awoke, the Mother called 911 and temp 101. When the ambulance arrived approx. 30 min. after the call, the Mother states the temp was 103. On the day that the 2nd flu shot was administered, the child was also ill with an acute otitis media and anemia and was being treated with omnicef 125 1 1/4 tsp /day and ferisol drops BID. Child in today for well child visit and appears well. Advised Mother to discuss incident with her child's physician before getting another flu shot for her child in the future. 03/23/2007 MR received for an ED visit for parental report of first seizure at home on the day of Flu vax. Child had been running a fever. His eyes rolled back and then began shaking all over. This lasted appox. 1 minute after which he awoke and took a bottle. PE: Crying and fretful. Temp=103'F. Skin feels hot. TMs red bilaterally. Otherwise WNL. TXd with Tylenol and Motrin with fever reduction. DC home. Discharge DX: Febrile Seizure.". "omnicef, ferisol. Meds: Motrin, Omnicef, Ferrous Sulfate". . Patient recovered.
