PNEUMO (PNEUMOVAX) problem was reported from WV on Feb 06, 2007. Female patient , 30 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Feb 06, 2007. "Pt c/o chest pains about 3-4 hrs. after shot. Then began with body aches,fever, chills, and errythemia of R upper arm the next morning. By Wednesday morning symptoms were worsening and family took her to ER when fingernail beds were turing blue. BP in ER 81/55. Md felt she was starting to go into shock. WBC count 24.9 in ER at 2 pm on 01/31/07. Admitted to Acute Care. WBC on 02/02/07 was 10.6. Highest oral temp in ACF was 101.3. 02/16/2007 MR received from hospital admission of 5 days for a progressive, worsening reaction to Pnumovax. Pt. presented to the ER with flu-like symptoms (fever of 101'F, dizziness, chills) and severe right shoulder pain. HPI: The evening following pneumovax, pt. began to experience ""chest tightening"". The following morning the arm was swollen and sore with decreased ROM. She had a temp. of 101' and felt dizzy. Later that day it was noted that she was experiencing nailbed duskyness which prompted the trip to the ER. BP upon arrival was 81/51. The right shoulder showed erythema and prominent tenderness upon PE. Treated with triple antibiotics with improvement. Impression: Severe allergic infectious process to the right shoulder after pneumovax. Cellulitis/abscess could not be ruled out.". . . Patient was hospitalized Patient recovered.

PNEUMO (PNEUMOVAX) problem was reported from WI on Feb 08, 2007. Female patient , 80 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Feb 08, 2007. Patient developed redness around injection site from the top of the shoulder extending down to the elbow. Area was sore and tender to the touch. Patient complained of pain on 2/7/07. Area was still sore on 2/8/07 and was noted to have the above mentioned erythema. "Levaquin 250 mg IV daily, furosemide, tobradex, albuterol inhaler, calcium, Vitamin B12, Vitamin E, folic acid, warfarin, acetaminophen, potassium, digoxin, cardizem, diltiazem, flovent, Restatsis eye drops, alphagan eye drops, flax seed oi". Administered oxygen 2L by nasal cannula. Rechecked BP 100/60, gave crackers and juice, pt responded well. Discussed side effects to monitor for at home w/mom. "Received Pneumovax approx 10:00 am, by 12:30 pm she noticed swelling, hotness, redness, pain and itching in left arm, fingers, breast area and into neck. Has gotten worse in pas 24 hours; has tried Aleve (Advil) with some relief. Dr. recommends by phone to taken Benadryl 25-50 mg every 6 hours as needed. PNEUMO (PNEUMOVAX) problem was reported from CO on Feb 16, 2007. Female patient , 37 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Feb 15, 2007. "Information has been received from a registered nurse concerning a female with no drug reactions of allergies and with reportedly immunosuppression due to multiple sclerosis, who on 06-FEB-2007 was vaccinated into the right deltoid with a 0.5ml dose of Pneumovax (lot #655152/0889F). The patient was concomitantly vaccinated with Dtap into the left deltoid. Concomitant medications included medications for multiple sclerosis. It was reported that the patient presented to the office yesterday (07-NOV-2007) with high fever 101, redness at the injection site which was the right deltoid, swelling measured 5 by 4 centimeters. The patient experienced extreme pain at the injection site and limited movement of her right arm due to the pain. The patient sought unspecified medical attention and was treated with oral antibiotic. The patient had a follow up visit and the site was 7 by 6 centimeters of redness, but the swelling had gone down and her temperature was 99.8. The injection site was less tender and the patient had more movement of the right arm. The reporter determined the patients high fever 101, redness at the injection site, swelling measured 5 by 1 centimeters, extreme pain at the injection site and limited movement of right arm due to pain were considered to be disabling and other important medical events. Patient complained of pain where vaccine administered. Site assessed and noted to be edematous (size of tennis ball per nurse). Ice compress applied and Darvocet N 100 given for pain. Physician was informed and a picture of the reaction was taken. "Aspirin, famotidine, promethazine, protonix, clonidine, proscar, plendil, furosemide, plendil, metoprolol, plavix, zocor". PNEUMO (PNEUMOVAX) problem was reported from NM on Feb 20, 2007. Female patient , 59 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Feb 13, 2007. Pt received a Tdap immunization in the R deltoid on 2/5/07. She had no reaction until 2/11/07. At that time she started itching and noticed two 3x3 inch area of erythema with a 1/2 inch area that connected them. Seen by Dr. No apparent infection. Spoke with pt today and it is itching and is fading. Not itching 2/13/07 and decreased erythema.. "Lovastatin, Atenolol, ASA, Calcium, Isosorbide,". "Hypertension, Osteoporosis.". Patient recovered.

"Information has been received from a 70 year old female consumer with rheumatoid arthritis, allergy to heavy metals including gold and mercury, penicillin, aspirin (ringing in ears), formaldehyde, iodine (injection and topical), raloxifene HCL, indomethacin, lisinopril, ezetimibe and alendronate sodium allergy, and latex/band aid allergy and a history of bone cancer (1989), left lower leg amputation and platinum containing chemotherapy who in October 2001, was vaccinated with Pneumovax. Concomitant therapy included vitamins (unspecified) and herbs (unspecified) including POLICOSANOL. In October 2001, the day after vaccination with Pneumovax, the patient developed Stevens Johnson syndrome. Symptoms included ""blisters, internal heat, and horrendous pain"". The blisters were the size of quarters on the patients torso. The patient was bed-ridden. Her skin was sloughing off and she became allergic to water. The patient wore cloth gloves under rubber gloves to avoid contact with water. When water touched the patients wrists, blisters would form. When the patient wore sleeveless shirts, the skin under her armpits and around her shoulders would rub off. The patient became ""delirious"" and experienced hallucinations. The patient was not certain whether she had hallucinated due to medications which were ordered to resolve her symptoms. The medications included prednisone, ATARAX and DECADRON. The patient also mentioned that she took ""nerve blocks"" for sciatica. After 3-6 months, the patient's symptoms resolved. Upon internal review Stevens Johnson syndrome was considered to be an other important medical event. The patient also mentioned that she experienced drug reaction with lisinopril (manufacturer unknown) NWAES0702USA03984, ezetimibe NWAES0702USA03994, indomethacin NWAES0702USA04037 and alendronate sodium NWAES0702USA03985.". . . Patient recovered.

PNEUMO (PNEUMOVAX) problem was reported from VA on Feb 27, 2007. Female patient , 45 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Feb 27, 2007. On 2/22/07 pt had half-dollar sized red area with white center & pain. 2/23/07 pt had red area size of a raquetball. 2/24/07 she had itching at the site but was smaller. 2/25/07 it red area was quarter-sized. 2/27/07 the redness is gone and swelling gone.. "Lexapro, Calcium + D, Motrin". "Muscular dystrophy, Hearing loss, Knee DJD, premenstrual syndrome". Patient recovered.

PNEUMO (PNEUMOVAX) problem was reported from CA on Feb 27, 2007. Male patient , 65 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Feb 26, 2007. "Pain, redness, swelling nausea, vomiting.". . NONE. Patient recovered.

"24 hours after vaccine running fever 101.1, with very sore and swollen injection site, and myalgia". Imetrex. "Imitrex 100 mg Prn migraine,Estradial 1mg 1/2 tablet daily,Ceftin 250mg 1 bid started on 02-27-2007". Patient recovered.

PNEUMO (PNEUMOVAX) problem was reported from MA on Mar 02, 2007. Male patient , 66 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Feb 26, 2007. "Pt received pneumococcal 23 on 2-20-07. Pt called out office on 2-21-07 complaining of left arm soreness, slightly red, swollen, itchy. Pt seen by pcp 2-21-07-subjective data=erythema overlying left elbow; tender in deltoid region but no fluctuance warmth or erythema with pt supine. Treatment cellulitis vs reaction to vaccine, placed on Dicloxacillin sodium cap 500 mg". "Omeprazole, Warfarin, Simvastatin, Metformin, Glyburide, HCT, Atenolol". "NKDA, DM type 2, CAD, MI, Nephrolithiasis, HTN". Patient recovered.

PNEUMO (PNEUMOVAX) problem was reported from TN on Mar 02, 2007. Female patient , 56 years of age, was vaccinated with PNEUMO (PNEUMOVAX) on Mar 02, 2007. "Received vaccine approximately 15:00 hrs, developed nausea, vomiting, fever of 101F around 19:00 hrs. The next day she returned to see MD, temp. 102.7F, erythematous patch with central induration around vaccine site, intermitten chills, fatigue, nausea and vomiting. Blood cultures and labs drawn, peripheral IV started, anzement and unasyn given. MD discussed case with Dermatologist, pt. admitted to Medical Center for further evaluation and treatment. 03/05/2007 MR received for a 3 day hospital stay for fever, nausea and vomiting, with inability to keep medications down which occured on the evening of her Pneumovax. PE showed a 3 inch erythematous patch on LUE with central 2cm. induration. Temp-102.7'F. Otherwise WNL. Pt. treated with antibiotics and discharged on day 3. Principal DX: Arthus Reaction. Secondary DXs: APF, HTN, GERD, Fibromyalgia, IBS.". "Inerferon Gamma-1B, lasix, prednisone,aspirin,nexium,plavix, cozaar,tylenol ES,caltrate plus D, zyrtec,xanax, nasacort AQ,fish oil, tylenol,nitroquick,cymbalta, sulfa trim DS,metoprolol,vytorin. Meds: Interferon-gamma. Lasix. Prednisone.". "idiopathic pulmonary fibrosis, fibromyalgia, depression, anxiety, irritable bowel syndrome, obesity, allergic rhinitis, hypertension, GERD, cervical fusion, coronary stent, myocardial infarction (2003). PMH: Idiopathic Pulmonary Fibrosis/U". Patient was hospitalized Patient recovered.
