Started 2 days after injection. Patient called doctor about pain. Doctor said possibly from administration. Patient came in to pharmacy today to pick up pain meds. Looked at arm 1/8 inch bruise at injection site. About 2 1/2 inch circumference bruise on upper posterior arm, when patient questioned, claimed been there for many months, by hitting a shelf. (Questionable: outer bruise, brown, yellowish, white center did not look new). Unrelated hospital visit because of knee abrasion looked infected. Switch Levaquin to Augmentin.". "VICODIN, AUGMENTIN, CYMBALTA, WELLBUTRIN XL, PROPANOLOL". "Hypertension, Depression". Patient recovered.

Patient also had a TB skin test, Mantoux Tuberculin, Lot # C2513AB, given in the left arm intradermal.". Sulfa and Latex Powder. Patient recovered.

Shortly thereafter began experiencing excruciatingly painful muscle and joint pain in all extremities and back as well as losing strength in arms hands and legs. Most intense pain was in upper arms and thighs. Lost all strength in hands and arms and went to urgent care on 6-25-07; given script for pain referred to my primary MD. Have been confined to bed since then.. Ambien CR 12.5 mg per daily; Cymbalta 30 mg 3 x per day. Osteoarthritis/Fibromyalgia/Degenerative Disk Disease/Depression. Patient recovered.

Client reports that on the second day after the vaccination she developed severe pain and weakness in the right arm where she received the Td injection. She describes the weakness as so severe that she had difficulty picking up light objects. She ran a fever of 101 degrees. She did not seek medical attention. The symptoms improved over a 4 day period. No long-term adverse effects remain.. "Lamictal, Celexa, Amitzia". "Penicillin, Bipolar Disorder, Irritable Bowel Syndrome". Patient recovered.

TD ADSORBED (NO BRAND NAME) problem was reported from TX on Aug 21, 2007. Female patient , 55 years of age, was vaccinated with TD ADSORBED (NO BRAND NAME) on Aug 21, 2007. "Client received a Td injection IM in left deltoid on 8/13/2007 following stepping on a piece of glass in her yard. On 8/15/07 she reported to another health unit with erythema, edema & local heat of the Left deltoid area and a headache. The next day she returned with the erythema, edema, heat which had spread distally from the deltoid area and a pustule at site of injection. She was referred to her MD but did not go. She returned here on 8/17/07 but the erythema and edema had subsided from deltoid but was still evident on upper arm near axilla. Client informed me that she had run a fever of at least 101 degrees for three days after receiving the vaccine. She was referred to her MD again and did go on this date. Her MD placed her on an antibiotic and anti-inflammatory medication and today she is much improved. She says she had good mobility in the arm during this episode and she did continue to come to work.". "Norvasc, Allegra, Nexium, Synthroid, HCTZ". "No known drug allergies, dust & molds". Patient recovered.

A 41-year-old female patient with a history of asthma, seasonal allergies, penicillin allergy, and varicose veins, received a right deltoid injection of Decavac (lot number U1953U) on 08 July 2007 for a ""dirty wound"". Two days later, on 10 July 2007, she began experiencing sharp pain in the left lower back, left eye ""fuzzy"". . . Patient recovered.

Right arm IM area redness and swelling present and right arm pain - still has right arm soreness.. "SPIRIVA HANDIHALER, ALBUTEROL INHALER". . Patient recovered.

TD ADSORBED (NO BRAND NAME) problem was reported from NJ on Sept 21, 2007. Female patient , 65 years of age, was vaccinated with TD ADSORBED (NO BRAND NAME) on Sept 17, 2007. "9/10/07 Extreme pain - slight swelling ate site. 9/11/07 Pain, swelling, site very hot. 9/12/07 Pain less but swelling more severe with itchiness. Swollen site about 2 inches. 9/15/07 Itchiness and swelling subsiding.". . . Patient recovered.
