INFLUENZA (H1N1) (H1N1 (MONOVALENT) SANOFI) vaccine side effect was reported on 10/22/2009. Female patient, child 1.5 years of age, was vaccinated with INFLUENZA (H1N1) (H1N1 (MONOVALENT) SANOFI). Patient symptoms: Muscle twitching, "Fever, irritability, seizure-like twitching, though no loss of consciousness, and no alteration in conciousness. Had normal CBC, electrolytes, CSF, normal CT of head, normal EEG, negative viral PCR panel. Pyuria (10 WBC - negative culture) Fever and twitching movements resolved after 3 days. 10/23/09 Hospital records received service dates 10/20/09 to 10/21/09. Assessment: Pyuria, resolving movement disorder with normal EEG. Patient presents with recent hx of mild febrile illness and seizure like activity. ~Crabby and sick~, runny bowel movement. Woke up screaming at midnight. Jerking of whole body, then screaming/crying for several minutes. Continued to have these events. Mild congestion, runny nose, cough. 10/29/09 Discharge summary received for DOS 10/20-10/22: Final DX: Spasms. Patient presented in ED with fever and spastic-like motions. Seizure concerns. Activity present during EEG. EEG and CT brain WNL. Discharged. Follow-up with PCP and possibly neuro as needed. 11/2/09 Medical records received for date 10/21/09 Neuro consult inpt. DX: febrile seizure, infantile spasms. Assessment: WNL other than h/o. Pt. received H1N1 vax on 10/16 then experienced per parents increase in fussiness, tactile fever, and 1 loose stool. On 10/19 per parents pt experienced a 5 sec. episode of generalized jerking before falling asleep. Since this episode pt has had several more episodes per parents that are characterized by bilat. Patient symptoms: Muscle twitching, "Approximately 1 hour after receiving H1N1 vaccine, her R hand felt very hot and tight but with no obvious tx measure change or swelling; this sensation progressed over the hours to her wrists, elbow and then had ~electrical jolts~ which made her arm twitch." INFLUENZA (H1N1) (H1N1 (MONOVALENT) SANOFI) vaccine side effect was reported on 10/29/2009. Male patient, child 3.0 years of age, was vaccinated with INFLUENZA (H1N1) (H1N1 (MONOVALENT) SANOFI). Patient symptoms: Muscle twitching, "Initial case was received on 26 October 2009 from a physician. This was reported as one of a cluster of two patients who experienced adverse events following vaccination with H1N1 2009 Monovalent Vaccine - Preservative (lot number UP001AA). The other patient experienced a non-serious event, which is captured in case number 2009-04481. A three-year-old male patient, with no reported medical history, experienced dystonia after receiving an intramuscular left deltoid first injection of H1N1 2009 Monovalent Vaccine - Preservative (lot number UP001AA) on 14 October 2009. Approximately four days post-vaccination, on 17 or 18 October 2009, the patient's mother noted right eye twitching, jerking of the right arm, and occasional jerking of the right leg. The symptoms occurred daily, particularly when the child was attempting to fall asleep. He was seen by his physician on 21 October 2009 and an exam was normal at that time; however, the physician noted that the patient's mother had videotaped the events and that the videotape was ~impressive~. He was subsequently seen in the emergency room, and was diagnosed with dystonia. An MRI and EEG were normal. At the time of the report, the event had not recovered. The patient had not received any other vaccines within four weeks of vaccination. Per the reporter, he did receive annual seasonal influenza vaccines. Patient symptoms: Muscle twitching, "Child received H1N1 vaccine, #1 dose, at 10:15 AM. Within 5 minutes patient dropped to the floor. This was not witnessed by an adult. Once observed by adult, within a minute, observer noted minor twitching of upper extremities. Heart rate was 80-88." . Patient recovered.

Patient symptoms: Muscle twitching, "Pt initally with HA and lethargy. School nurse reports she looked gray ~like she needed oxygen~. Then 2-4 hours later she developed dytonic reactions with back arching and mucscle spasms that resolved with IV benadryl. She was admitted to Medical Center at 3am on 11/5/2009. She did well until 3:30pm when she again began having muscle twitching which is lessening after IV benadryl. With the parents permission I video taped these movements. Of note, the school nurse reports a large percentage of student (estimated >25%) had c/o HA and lethargy after their H1N1 immunization. 11/9 and 11/12/2009 hospital records for 11/4-11/6/2009.Dc Dx dystonia ICD-9 code 333.72 Patient with c/o's dizziness, tremors. Tx: IV Benadryl. Neuro exam was normal, neuro consult indicated that MD found no neurological basis for patient's c/o's. 11/19 and 11/25/09: Hospital records and neurology consultation received for dates of service 11/4-11/6/09: Dx: dystonia vs. chorea vs. conversion reaction. Assessment: Neurology consult and examination find no chorea, athetosis, dystonic reaction or akathisia. MRI normal. There is no LOC with events. Dr. suggests converstion type reaction. Patient symptoms: Muscle twitching, "Fingers and hands and toes and feet tingled, involuntary ~twitched~ ... symptoms woke me up and I had to walk around and shake / grip my hands, laid back down, and had to get up again ... intensity did not vary and persisted all night - especially when I laid down, as it was not as pronounced when I was walking around and shaking / gripping my hands. Felt sleep deprived the next day and the next night the same thing occurred. I almost went to my personal physician / ER, but did not and then reported the symtpoms to the OST Health Unit on 11/02/09. The symptoms essentially went away from 11/02/04 until the night of 11/04/09, when they came back and woke me up again. I have not experienced the symtpoms since that time." Patient symptoms: Muscle twitching, "approx. 2 min. post injection, I saw her standing nearby me and she just went down onto the floor. I caught her and she didn't hit head. Had very brief seizure-like movement rt. arm &leg. She then awoke and wanted to sit up. Had circum-oral pallor, lasting 10-15 min. 911 was called. She remembered me giving her a shot and she went to sleep, omly asking for a drink of water. All this was least than a minute. Ambulance did take her to the E.R. and mother was called. 11/17/09 ED records received service date 11/12/09. Assessment: Syncope - vasovagal. Patient stated she was apprehensive, received vaccine at school, lost consciousness less than 10 seconds, brief right arm and right leg twitching. When she regained consciousness she was immediately alert and not confused. Presents at ED happy and alert. No other complaints." Patient symptoms: Muscle twitching, "Initial report received on 10 December 2009 from a consumer. A 42 year old female patient with a history of CROHN'S disease (treated with mercaptopurine) developed muscle pain in her right thigh six days after receiving a right deltoid intramuscular dose of Influenza (H1N1) 2009 Monovalent Vaccine (lot number UP002AA). The day after the muscle pain began, the patient developed twitching in her legs which became widespread, including her lips. The patient also complained of pins and needles in her feet and other locations on her body. The patient went to the neurologist and was diagnosed with neuropathy. Blood work and nerve conduction study were completed, however the results were not provided. The patient was reported as not recovered. List of Documents held by the Sender: none." Patient symptoms: Muscle twitching, Muscles got tense and a creepy crawly sensations around the arm and up the shoulder and around to the back. Next day my eye was twitching. The next day I developed Bells Palsy. It was confirmed by my doctor who gave me meds and I have a referral to the neurologist Jan. 21st. NoneNone . Patient recovered.
